Sample Deposit


Lift toilet seat, secure disposable collection paper on the rim of toilet bowl, lower lid.
Make bowel Movement onto collection paper.
See illustrations for "Sample Collection".After sample cllected, release adhesive tabs and flush disposable collection
Place specimen tube into zip bag and then put that with return form into supplied mailing envelope and seal.
Address mailing envelope to your doctor or laboratory.
Return the sample package immediately to the clinic or laboratoty by mail or personal delivery.
Sample Collection


1. Unscrew the cap from the sample collection tube.
2. Poke special applicator into stool at six different sites.
3. Screw theapplicator back into the tube and secure tightly.
Test Procedure

1. Shake the sample collection tube well.
2. Open pouch and place test device on a flat surface
3. Open the tip cover of the sample collection tube and squeeze tube to dispense three drops into sample well.
Read result at five minutes. DO NOT INTERPRET RESULT AFTER TEN MINUTES.
